HEALTH COORDINATOR



Purpose
To ensure that health needs of Persons of Concerns (PoCs) in the urban areas in Lusaka and referred patients from the refugee settlements are provided through linkages with appropriate health facilities in urban Lusaka. To enhance and improve communication and coordination among other implementing partners and Partner’s offices on the proper referral of patients to Lusaka and back to the refugee settlements. To ensure that referred patients staying in Chilenje house and Makeni Transit center are receiving proper treatment and care including prescribed drugs ( medicines) which are not readily available in the hospitals and clinics.  To ensure that Chilenje house and Makeni TC operate according to relevant standard and that relevant Standard Operating Procedures are in place and followed (e.g. SOPs on medical referrals). To ensure improved coordination with the Ministry of Health in relation to provision of health services to PoCs. To ensure support to urban refugee project activities on health and Partner’s protection programs such as, but not limited to, participation in the Urban Residency Committee (REC-Office of the Commissioner for Refugees) and attending protection panel meetings, and accomplishment of Medical Assessment Form (MAF) for resettlement and urban on medical ground.
Key Responsibilities
Under the direct supervision of the Urban Refugee Project Manager with technical supervision of Partner’s Public Health Officer, the Health Coordinator will undertake the following tasks:
  • Take a lead role in ensuring that PoCs in the urban areas are informed and aware that they can access health services in the different government clinics/facilities within their respective communities through sensitization activities and community outreach.
  • Link and facilitate referral of  PoCs requiring special treatment to the appropriate higher level of health care following the national referral system and the Partner’s Guidance and SOP on medical referral
  • Receive and properly document patients referred to Lusaka and make sure that they have proper referral letters/notes and other relevant documents (medical records)
  • Establish appropriate record/inventory on patients referral which includes information on clinical diagnosis, days of hospitalization, procedures/surgeries done, and  cost incurred(assistance from The Partner) and submit monthly report to the supervisor/Urban project and providing copy to the Partner’s Public Health Officer
  • Provide monthly summary inventory/audit of patients staying in Chilenje house/Makeni Transit Center including patient received and mobilized back to the settlements.
  • Oversees the follow up of all patients to ensure that they get the intended care. This entails arranging appointment, accommodation, transport, and regular visits to refugees hospitalized.
  • Provide regular (weekly) feedback to the Referring Officer and to the Partner on all referred cases to Lusaka.
  • Discuss with the health provider/attending physician the possible continuation of treatment in the health centers in the settlements or in the lower health care facilities without necessarily returning the patient back to Lusaka for follow-up reviews unless it’s really necessary. Ensure that patients will be issued a medical report/certificate upon discharge from the hospital or before returning to the refugee settlements.
  • Participate in regular coordination meetings with Donors and other Partners including participation in sector meeting organized by AAHZ.
  • Attend, participate, and contribute in any forum that needs medical expertise such as, but not limited to, the Partner’s protection panel meetings and Urban Residency Committee (REC-Office of the Commissioner for Refugees) meetings. He/she will review records of individual cases with medical needs and will share his expert opinion and recommendations to relevant panels in order to facilitate decision making.
  • He/she will work in collaboration with the Partner and AAHZ’s Community Services staff in identifying, assessing and referring individual cases of Persons of concern with specific health needs to relevant protection panels (Partner’s Protection Panels, REC, etc.) through submission of relevant documentation.
  • He/she will act as focal point in the timely accomplishment of the Medical Assessment Form (MAF) for resettlement in medical ground. He/she will coordinate with the Partner’s resettlement unit and the independent medical practitioner(s) who will accomplish the MAF and make sure that the MAF is complete with all the necessary information and submitted to the resettlement unit in a timely manner.
  • He/She will liaise and coordinate with the Ministry of Health for any relevant health and medical issues related to PoCs
  • He/she will provide supervisor with regular post-field visit and/or post-meeting reports (1-2 pages – bullet points on key observations, findings, etc.)
  • Provide inputs on program planning and report development.
  • Submit regular (monthly) reports to the project/supervisor or as required.
  • Perform other tasks as required by the supervisor
Results/Expected Output:
  • Quality primary health care and other public health services are available and accessible to all refugees and people of concern (POC);
  • Health and medical status of persons of concern to Partner improved;
  • Record of patients referred/referral log book with comprehensive information is established and updated;
  • Periodic reports of good quality are submitted on time: (refer to key responsibility # 4,5, 14);
  • Sensitization activities for PoCs on health issues, availability and access of health services conducted; Health promotion materials are developed and disseminated;
  • Understanding and application of the partner’s Guidelines and SOP on medical referral
  • Referral of PoC’s in urban areas to appropriate higher level of health facilities facilitated;
  • PoCs with specific medical needs are identified and referred to relevant panels
